When your schedule is erratic, this clock breaks down, leaving you wired at night and exhausted during the day.

Establish Your Morning Anchor
The biological clock is anchored to the morning. Many patients spend immense mental energy trying to control the exact moment they fall asleep, which is often the hardest point of the cycle to regulate. Instead, I instruct my patients to focus entirely on their wake time.
You must rise from bed at the exact same time every single day, including weekends and your days off. Consistency trains your body to expect rest at a predictable interval and reinforces your sleep-wake cycle. Even if you have a terrible night of sleep and feel completely exhausted, you must resist the urge to sleep in.
To reinforce this anchor, get bright sunlight exposure within the first hours of your morning. Spend thirty minutes outside in the sun. Even cloudy-day light is significantly stronger than indoor lighting and helps set your circadian rhythm, promoting alertness during the day and deep sleep at night.

It destroys rapid-eye-movement cycles, prevents deep restorative sleep, and often causes middle-of-the-night awakenings. If you choose to drink, finish your beverage several hours before bed.
- Meals and Liquids: Avoid consuming heavy, large, or spicy meals within three hours of bedtime to prevent active digestion from keeping your core temperature elevated. Additionally, limit liquid intake in the evening to avoid waking up for nighttime bathroom trips.
The Twenty-Minute Rule and Worry Management
When sleep does not come easily, lying in bed tossing and turning is highly counterproductive. It creates a negative psychological association, training your brain to view your bed as a place of frustration and anxiety.

A dark, quiet bedroom often invites unwanted worries. To prevent your mind from racing at midnight, schedule a dedicated twenty-minute worry time in the afternoon. Sit in a room other than your bedroom, write down your concerns, plan your solutions, and then physically set the paper aside. Tracking and Adjusting: Sleep Efficiency
In clinical practice, we measure the quality of your rest using a metric called sleep efficiency. This is the percentage of time you spend asleep while in bed:
Sleep Efficiency % = (Total Time Spent Asleep ÷ Total Time Spent in Bed) × 100
An ideal sleep efficiency is 85 percent or higher, meaning you are asleep for the vast majority of the time your mattress is occupied. If you have a smart watch or other way to track your sleep quality, that is very helpful data as well. Garmin, Samsung, Apple and Google all make quality watches with sleep tracking capabilities that give actionable data for us to review together.

In cognitive-behavioral interventions for insomnia, we use this percentage to adjust your allowed schedule. This structured approach is adapted from the clinical protocols of the Centre for Clinical Interventions. If your sleep efficiency is low, we temporarily restrict your allowed time in bed to match your actual sleep capacity, biological sleep drive. As your sleep becomes more consolidated and your efficiency rises above 85 percent, we gradually increase your allowed time in bed.
Clinical Safety Note: If you have a diagnosed mood disorder, such as bipolar disorder, sleep restriction must be handled with extreme caution. Restricting sleep below 6.5 hours can trigger hypomania or mania, so any scheduling adjustments must be managed under professional clinical supervision.
